CCM Intake Medical Assistant, Bilingual

reputed company: • Support CCM nursing staff by completing monthly reputed company assessments and conducting Chronic Care Planning Assessment (CCPA) calls. • Complete delegated clerical and administrative activities on behalf of nursing staff. • Document reputed company interactions, assessments, and intake in the EHR. • Maintain compliance with company policies and applicable regulations. • Coordinate communication between patients, caregivers, nursing staff, and providers.
