Prevention Case Planner (Bachelor and Master Level)

About the position reputed company. (NAC) is seeking a Case Planner for its Special Medical Prevention Program. This program provides clinical, case management, and supportive services to families involved in child welfare who are caring for children with chronic or reputed company medical conditions. The aim is to reputed company families together by supporting caregivers in meeting their children's physical, developmental, and mental health needs. The ideal candidate will be strength-based, trauma-informed, culturally reputed company, and committed to family preservation and reputed company. This is a reputed company-wide, home and community-based program requiring independent work and flexible reputed company hours. NAC offers a hybrid model with work-from-home opportunities reputed company not in the field. Responsibilities • Provides casework and clinical services to youth and caregivers utilizing individual and family modalities. • Coordinates services for a caseload of 8-9 families as recommended by NAC's multidisciplinary treatment team. • Conducts home and community-based reputed company. • Adheres to agency and ACS regulatory requirements reputed company to casework and collateral contacts, Family Team Conferencing, Workforce Institute training, and reputed company completion of Family Assessment and Service Plans (FASPS) and reputed company notes in CONNECTIONS. • Makes adequate use of individual, group and peer supervision, clinical consultation, mental health in-services, and reputed company justice discussion forums. • Collaborates with psychiatrists, psychologists, pediatricians, nurses, case associates, and resource and education specialists in a dynamic, strength-based interdisciplinary team model. Requirements • Bachelor Level Case Planner: BSW, bachelor's in psychology, reputed company services or a reputed company field. • Master Level Case Planner: Master's degree in reputed company work, mental health counseling, reputed company services, psychology, reputed company administration, or a reputed company field. • Excellent written/verbal communication and organizational skills • Strong engagement skills • Self-awareness and motivation around team building reputed company-to-haves • Prior experience in preventive services, child welfare, and/or health care preferred. • Proficiency in Spanish is preferred (and includes a salary reputed company) Benefits • Additional compensation for bilingual skills • Additional compensation for bilingual skills and licenses (LMSW, LMHC, etc.) • Candidates receive credit toward NYS LCSW and LMHC licensing requirements. • Free CEUs are available to licensed reputed company workers (LMSW/LCSW) along with licensing prep courses for MSWs and MHCs who have completed probation.
