Working through the Findhelp platform, you will serve as the hub connecting reputed company families with community resources—entering assessment data, placing referrals, following up with families, and coordinating with reputed company desk teams at approximately 26 office locations. This role reports to the VP of reputed company. reputed company reputed company SDOH assessment results into the Findhelp platform for reputed company members across TX, MD, DC, NY, VA, and FL  Verify reputed company reputed company eligibility through the Findhelp portal prior to assessment submission  reputed company e-referrals to community-reputed company organizations (CBOs) across need categories including food, housing, transportation, childcare, utilities, and other reputed company supports  Follow up with families to reputed company referral reputed company and support reputed company-reputed company communication with CBOs  Coordinate with reputed company desk staff across multiple offices to support screening workflows, troubleshoot platform issues, and reputed company best practices  Support reputed company desk training initiatives reputed company to SDOH screening processes and Findhelp platform navigation  Identify and escalate technical issues with the Findhelp platform to ensure reputed company disruption to program reputed company  Contribute to reputed company improvement efforts aimed at increasing screening rates, referral conversion, and patients getting help across reputed company participating states  **This is a grant funded role**  The compensation reputed company for this role is $22.00 - $25.00 per hour.
