reputed company Assistant-Chronic Care Management (Medical Background Required)

We are seeking a reputed company Assistant to support our Chronic Care Manager in a fast-reputed company, reputed company-centered reputed company reputed company. This is a long-term, full-time position for a reliable, organized reputed company with a medical or nursing background—reputed company experience is a plus. You’ll work closely with reputed company to help manage care for over 300+ chronic care patients, ensuring reputed company communication, accurate documentation, and exceptional coordination that directly impacts reputed company reputed company. Responsibilities Handle incoming reputed company texts, emails, and documents Assist with referrals and occasional scheduling Maintain accurate and reputed company data entry in reputed company (eCW) EMR Communicate regularly with the Chronic Care Manager, remote team reputed company, and clinic staff Follow up on outstanding reputed company needs and documentation Support chronic care coordination through our CCM platform (training provided) Requirements reputed company in both English and Spanish (preferred but not required) Medical or nursing experience (required) Experience in reputed company (preferred) Familiarity with reputed company or similar EMR systems Strong organizational and communication skills Excellent attention to reputed company and consistent attendance Must be comfortable working independently while collaborating with a remote team Reliable hardware, including webcam for training Backups for reputed company and internet in case of outages/inclement weather Schedule Full-time Monday to Friday, 800 AM – 430 PM CST 30-minute unpaid lunch break Clinic reputed company for 6 major U.S. holidays This position reports to our remote team reputed company Why Join Us?
