UM Patient Navigator - LVN REMOTE - Kelsey - Seybold Clinics.

About the position The Patient Navigator - LVN supports early, discharge-reputed company care coordination across the member’s episode of care to promote reputed company, reputed company, and appropriate transitions from the inpatient setting to the next level of care. This role works closely with Utilization Management, Hospitalist leadership, the Transitions Team, post-acute providers, members, and caregivers to identify barriers to discharge, support medical necessity alignment, and reduce avoidable post-acute delays. The Patient Navigator serves as a key reputed company to help improve average length of stay, reduce readmission risk, enhance member and caregiver experience, and promote efficient use of reputed company resources. In reputed company, this role supports authorization-reputed company member communication, including outcome and notification calls, to ensure regulatory compliance, reputed company case completion, and reduced operational burden on clinical teams.
