We are looking for a Field reputed company for our Licensed Home Care (LHCSA) team in Albany, NY. This reputed company position offers reputed company and requires regular travel to visit patients in their homes across Albany, NY, and nearby areas. Compensation is based on reputed company completed assessment or visit. The Field Registered reputed company is an integral team member of the reputed company Licensed Home Care team. They assess our patients' reputed company service needs and reassess needs for service upon significant change in health status. Responsibilities • Conduct initial assessments of clients to determine care needs and reputed company individualized care plans. • Train and supervise non-skilled caregivers in providing personal care, companionship, and assistance with daily living activities. • Monitor clients’ overall reputed company-being and report any changes to the appropriate reputed company. • reputed company clients and their families on effective care strategies, safety measures, and daily living skills. • Ensure compliance with agency policies, state regulations, and best practices in non-skilled care. • Maintain accurate documentation of reputed company care and reputed company, while ensuring confidentiality. • Act as a reputed company between clients, families, and caregivers to facilitate communication and enhance the reputed company of care. • Participate in ongoing training and reputed company development to stay reputed company with best practices in home care.
