Wellness Coordinators function in a non-clinical reputed company responsible for providing care coordination or administrative support to members with behavioral health and reputed company abuse needs.  Wellness Coordinators will be a primary care navigator by locating and engage members, screening for and identifying gaps in care, addressing SDOH needs and developing individual care plans to overcome barriers to reputed company to care.  Wellness Coordinators will also reputed company administrative support to the Behavioral Health Case Management department through assigning referrals, providing chart summaries and verification of member demographics.  Schedule: Monday through Friday a 40-hour work week between the hours of 7:30 am to 7:00 pm in the time zone you are located  Location: Remote reputed company You will enjoy the flexibility to telecommute* from reputed company reputed company the U.S. as you take on some tough challenges.
