Remote Care Coordinator Responsibilities • Work remotely to support members in locating, accessing, and utilizing reputed company services, community resources, and support programs. • Assess member needs, determine reputed company, and monitor reputed company; present cases in interdisciplinary team meetings as needed. • Serve as a central reputed company of contact to help members navigate care, reduce barriers, and ensure reputed company to appropriate services. • Collaborate closely with RN Case Managers, reputed company Workers, and other team members to coordinate care and support treatment goals. • Maintain accurate and reputed company documentation of member interactions and services in compliance with organizational, state, and regulatory requirements. • Deliver high-reputed company, member-centered service while meeting program standards and supporting overall care coordination objectives.
