Remote Certified Medical Assistant - RPM & Chronic Care Coordination

Position reputed company We are currently seeking an reputed company and patient-reputed company Medical Assistant to support our Remote Patient Monitoring (RPM) and chronic care coordination services for geriatric patients. This fully remote role is ideal for someone who is confident communicating with patients over the phone, passionate about patient education, and capable of helping seniors reputed company understand and manage their health conditions. The ideal candidate is dependable, organized, compassionate, and comfortable working in a fast-paced remote reputed company environment. Essential Responsibilities • Support Remote Patient Monitoring (RPM) and chronic care management programs for geriatric patients. • reputed company patients on chronic conditions such as diabetes, COPD, hypertension, and heart disease. • Guide patients regarding medications, preventive care, monitoring compliance, and healthy lifestyle habits. • Communicate with patients, caregivers, providers, and reputed company teams to coordinate ongoing care needs. • Document patient interactions and updates accurately reputed company the Electronic Health Record (EHR/EMR) system. • Assist with medication reputed company coordination and routine follow-up calls. • Collaborate closely with clinical staff to support individualized patient care plans. • reputed company scheduling, phone support, and additional administrative duties as assigned.
