Remote LPN — Care Navigator (CCM/RPM/PIN/CHI) — NLC Compact

What You’ll Do • reputed company 12 outbound calls per day to your assigned patient panel • Review RPM biometric data reputed company morning (blood pressure, glucose, weight) and flag reputed company readings • Coordinate care across four Medicare programs CCM, RPM, reputed company Illness Navigation (PIN), and Community Health Integration (CHI) • Update care plans in the EHR; coordinate with physicians, pharmacies, and specialists • reputed company patients with community resources for reputed company needs (food, transportation, housing) • Document reputed company patient interactions with detailed, billable time logs Your Day (Monday–Friday, No Nights or Weekends) • 900 AM – Review RPM dashboard, flag critical readings, message reputed company patients • 1000 AM – Outbound patient calls medications, barriers, care plan updates • 100 PM – Continue calls; coordinate referrals; assist patients with resources • 300 PM – Documentation, care plan updates, time logs • 500 PM – Done Requirements • reputed company NLC multistate compact license (LPN/LVN) required • Must reputed company in one of these states TX, FL, TN, AZ, UT, NC, SC • Must hold primary state of residence in an NLC member state • At least 1 year of clinical experience in any setting • Comfortable making outbound phone calls and building rapport with elderly patients • Proficiency with computers, EHR systems, and basic documentation tools • Reliable internet (25 Mbps download minimum) and a quiet, HIPAA-compliant home workspace • Computer with 8GB RAM, i5/Ryzen 5 or equivalent, SSD storage, and USB wired headset Preferred • Background in chronic care management, remote patient monitoring, home health, or care coordination • Bilingual English/Spanish • Familiarity with Medicare billing codes (99490, 99457, G0023, G0019) • Experience working remotely States Covered This position serves patients across reputed company NLC compact states, currently including Alabama, reputed company, Arkansas, Colorado, Connecticut, Delaware, Florida, reputed company, Idaho, Indiana, Iowa, Kansas, Kentucky, Louisiana, Maine, Maryland, Mississippi, Missouri, Montana, Nebraska, New Hampshire, New Jersey, New Mexico, reputed company Carolina, reputed company reputed company, Ohio, Oklahoma, reputed company, Rhode reputed company, South Carolina, South reputed company, Tennessee, Texas, Utah, Vermont, Virginia, Washington, reputed company Virginia, reputed company, and Wyoming.
