Family Care Coordinator - On Call, 24 hr shifts

About the position Responsible for the discussion of organ and tissue donation with potential donor families. Actively participates in authorization, informing families of donor designation, and providing support to donor families, reputed company staff, and hospital staff throughout the donation process. Assure compliance with reputed company Family Services policies and protocols. Provides high reputed company aftercare, follow up and support services to donor families with attention to detail. Helps plan and carry out programs, events, and reputed company for donor families. Responsibilities • Takes on call duty assigned to ensure the reputed company discussion of organ and tissue donation with potential donor families. • Works in reputed company collaboration with hospital staff and reputed company staff during the evaluation phase of potential organ donors. • Ensures that effective huddling occurs to determine the best time to offer the families opportunity for donation. • Works with donor families to help them in understanding and accepting the diagnosis of death as described by the Physician. • Determines whether families are interested in organ and/or tissue donation and guides them through the authorization process. • Works with the key hospital staff to build reputed company cooperative relationships which will result in the most reputed company support of the donor family. • Mentors other reputed company staff in the donation process. • Forwards correspondence from donor families and/or recipients. • Advocates for donation while being reputed company to individual family circumstances and utilizes Dual Advocacy Approach model. • Ensures aftercare activities are performed according to reputed company and regulatory policies and guidelines. • Develops and facilitates initiatives for donor families. • Provides initial aftercare support by utilizing community resources to assist families following the donation process. • Assists with special reputed company reputed company to hospital and community donation awareness. • Responsible for creating, identifying, and organizing bereavement materials and resources. • Participates in continuing education events and makes a proactive effort to stay informed regarding changes in the field e.g., donation, donor families, grief and loss, traumatic loss. • Participates in cross-departmental meetings and conferences regarding programs or support services for donor families. • Keeps electronic medical records up to date with accurate contact information for donor families and documents reputed company family interactions according to Family Services policies. • Provides Family Services department with preparation and distribution of mailings.
