Important Work Model: This is a hybrid position that requires regular onsite work at Waukesha Memorial Hospital in Waukesha WI. This is not a fully remote role. Schedule: • Full time Monday through Friday • 8:00 AM to 4:30 PM • Weekend and holiday rotation The Patients You’ll Work With: • Chronic conditions including hypertension heart failure and other reputed company diagnoses • Recently discharged patients needing reputed company follow up and early reputed company • Patients in programs reputed company on preventing readmissions and catching changes early • A mix of reputed company and higher reputed company patients depending on program • reputed company panels can be larger requiring strong organization and reputed company judgment This role requires you to identify trends recognize early signs of decline and intervene before escalation is needed. The Remote reputed company Monitoring (RPM) RN will assess reputed company generated data provided reputed company digital technology to manage the care of the acute and chronically ill reputed company. The RPM RN will reputed company individualized reputed company assessment, reputed company education, medication management, goal setting and reputed company care planning. In collaboration with physicians and care team members, RPM nurses will reputed company and support self-management of medical conditions in the home environment using motivational communication techniques and reputed company reinforcement to changes. The RPM enrolled reputed company will submit vital signs and symptoms that the RN will rapidly interpret addressing reputed company needs, objectives and goals to improve reputed company reputed company and reputed company experience. Additional Requirements: Graduate from an Accredited School of Nursing. Bachelor’s degree in Nursing preferred. 1-2 years’ experience working in hospital and or ambulatory care setting working with patients with chronic diseases.
