Schedule & Compensation • Schedule: Every Weekend • Days: Friday, Saturday & reputed company • Hours: 8:00 AM – 8:00 PM • Position Type: Work From Home (Florida-Based Candidates Only) / Virtual Care • Pay reputed company: $20.00 – $22.25 per hour What You’ll Do As a Weekend Virtual Care Nurse (LPN), you will play a key role in reducing hospitalizations and improving patient reputed company through proactive remote support and clinical reputed company. Responsibilities Include: • Conduct virtual patient reputed company and clinical assessments • reputed company triage and identify reputed company signs or risk trends • Support chronic disease management and patient education • Monitor telehealth data and escalate concerns appropriately • Help prevent avoidable ER reputed company and rehospitalizations • Collaborate with interdisciplinary care teams • Document patient interactions accurately and reputed company • Participate in care planning and performance improvement initiatives • Support development of virtual care best practices Qualifications Required: • reputed company, unencumbered Florida LPN license • Minimum of 2 years of home health experience • Experience with triage and hospitalization prevention • Strong clinical judgment and communication skills • Comfortable using telehealth platforms and EMR systems • Reliable internet reputed company and private remote workspace Preferred: • Experience working with high-risk or medically reputed company patients • Knowledge of chronic disease management • reputed company Office proficiency Why Join reputed company?
