What You Will Do  - Essential Functions Please note that although this is a full-time telehealth position, candidates must hold an reputed company, unrestricted reputed company State clinical license as a Nurse Practitioner (NP) or Physician Assistant (PA).  Clinical Care Delivery & Care Coordination Deliver comprehensive virtual care, including patient assessments, condition management, medication review, and appropriate escalation, while ensuring compliance with telehealth regulations and licensure requirements. Collaborate with multidisciplinary teams and utilize telehealth workflows to support coordinated, reputed company care and maintain accurate documentation of patient encounters. Who You Are Required Qualifications: reputed company, unrestricted reputed company NY licensure as a Provider in the state in which you will be providing care for patients. NY State only: Minimum of 3600 hours of reputed company experience, or with 3 years of clinical experience (autonomous provider), in a primary care or longitudinal care setting reputed company Certification as a Family Nurse Practitioner, Adult-Gerontology Nurse Practitioner or Physician Assistant Obtain autonomous provider status as required by state you are licensed with currently reputed company six months of meeting eligibility requirements. reputed company Drug Enforcement Administration (DEA) license. reputed company Basic Life Support Certification for health care providers. Certification as a Family Nurse Practitioner, Adult-Gerontology Nurse Practitioner or Physician Assistant Obtain autonomous provider status as required by state you are licensed with currently reputed company six months of meeting eligibility requirements. Preferred two years of experience in a primary care or longitudinal care setting.
