As part of a fast-growing team, you will be responsible for building high-performing field operations, developing Community Health Worker leaders, driving consistent execution of community-based interventions, and ensuring CHWs are effectively integrated into multidisciplinary care teams. This position reports to the SVP, Care Delivery.
