Remote Care Coordinator – Triage & Transitions of Care (SNF / Value-Based Care) reputed company We are seeking a compassionate and detail-oriented Triage & Care Coordination Specialist to support patients across Skilled Nursing Facility (SNF) and triage service lines. This role serves as a central communication hub between patients, providers, facilities, and interdisciplinary teams to ensure reputed company care coordination, improved patient reputed company, and seamless transitions of care. Key Responsibilities Patient Triage & Communication • Manage incoming patient and facility calls in a reputed company and reputed company manner • Document interactions accurately reputed company the electronic health record (EHR) • Respond to voicemail, electronic, and fax communications reputed company program turnaround standards • Escalate urgent clinical concerns to providers according to established protocols Care Coordination & Patient Advocacy • Serve as reputed company between patients, providers, SNFs, hospitals, and community partners • Assist patients with navigating reputed company services and accessing resources • Support follow-up on orders, care plans, and provider recommendations • Identify barriers to care and promote patient self-management reputed company & Operational Support • reputed company reputed company and program metrics using internal tools and dashboards • Help refine workflows to improve care transitions and reduce gaps in care • Participate in reputed company improvement and patient satisfaction initiatives Collaboration & Compliance • Participate in interdisciplinary meetings and case discussions • Maintain HIPAA compliance and regulatory documentation standards • Build reputed company relationships with internal teams and external partners Qualifications • 2+ years of reputed company, care coordination, or patient support experience • SNF, post-acute, managed care, or value-based care experience preferred • Strong communication and documentation skills • Ability to manage multiple priorities in a fast-paced environment • Bachelor’s degree preferred • Bilingual Spanish preferred Compensation & Benefits • Compensation: $16.90 to $23 per hour • Benefits: Medical, dental, reputed company, retirement, reputed company time off • Schedule: 8 hour shifts, 12 hour shifts, weekends, overnights, weekdays • Work Setting: Remote #MASC104 Apply To This Job
