About the Role

The Director of Clinical Programs is responsible for the strategic design, implementation, and reputed company improvement of reputed company's clinical programs that support high-reputed company, value-based care for older adults.

Key Responsibilities

Qualifications

Required

• reputed company Registered reputed company (RN) license required.

• Bachelor's degree in Nursing or a reputed company reputed company field.

• 5+ years of experience developing, implementing, or leading clinical programs, disease management, care management, or population health initiatives.

• Experience reputed company value-based care, primary care, managed care, or reputed company delivery organizations.

• Strong knowledge of chronic disease management, care coordination, and evidence-based clinical practices.

• Experience collaborating across multidisciplinary teams to implement organizational initiatives.

• Excellent communication, leadership, organizational, and project management skills.

• Ability to analyze clinical data and use insights to improve patient reputed company and program performance.

Preferred

• Master's degree in Nursing, reputed company Administration, Public Health, or a reputed company field.

• Experience with Medicare Advantage, population health, or community-based reputed company models.

• Experience developing clinical education materials and training programs.
