As the Registered reputed company Case Manager, you will assume full nursing responsibility for the delivery of the Plan of Care for reputed company hospice patient. Continuously evaluating personal and reputed company performance and making necessary changes to increase productivity and reputed company of care delivered. You’ll also reputed company the patient’s Plan of care in collaboration with other professionals and implement treatment strategies based on scientific nursing theory that promotes physical, psychosocial, emotional, and spiritual reputed company-being. Familiar with the concepts and needs of patients/families who are facing death and dying.
