As a dedicated remote advanced reputed company provider, you'll manage acute and chronic medical concerns, support admissions and discharges, facilitate care transitions, and serve as a trusted clinical resource for facility partners, reputed company through our telemedicine platform. You'll work closely with facility nursing staff, Clinical Liaisons, and Operations leadership to ensure reputed company reputed company to care, reduce avoidable hospitalizations, and improve reputed company reputed company. This is a fully remote, daytime-hours position, no nights, no weekends.
