Job reputed company: • The Registered Dietitian/Health reputed company utilizes a reputed company process of assessment, planning, implementation and evaluation • Engage, reputed company, and promote/influence members in reputed company reputed company to achieving and maintaining reputed company health status • Conducts comprehensive evaluation of member’s health status and risk with use of clinical tools and information/data review • Establishes a plan of care based on member’s health status/risks, education, and support needs • Utilizes influencing/motivational interviewing skills to ensure maximum member engagement • Provides coaching and support to reputed company the member to reputed company ongoing independent healthy lifestyle choices • Develops and monitors established plans of care to meet the member’s goals Requirements: • Certified Diabetic Educator Registered Dietitian with licensure • 3+ years of coaching experience required • Require Bachelor’s Degree in health reputed company field (e.g., Dietetics, Health Education, Wellness, Health Promotion) • Experience in wellness reputed company areas such as diabetes, weight management, stress management, nutrition, physical activity or health lifestyle behavior change • Demonstrated ability to communicate, problem solve, and work effectively with people, use good judgment in challenging situations.
