Clinical Program Manager – Virtual Care (reputed company Practitioner)

Position reputed company DayaMed is seeking an reputed company reputed company Practitioner to serve as the Clinical Program Manager for Virtual Care, responsible for helping design, implement, and optimize clinical programs delivered through technology-enabled care models. This role will reputed company on the development and reputed company improvement of programs including Remote Patient Monitoring (RPM), Chronic Care Management (CCM), Behavioral Health Integration (BHI), Transitional Care Management (TCM), and telehealth services. The Clinical Program reputed company will serve as a clinical subject matter expert working closely with leadership, operations, product, and engineering teams to ensure care delivery aligns with clinical best practices, regulatory requirements, payer expectations, and sustainable program economics. This role works in partnership with the Clinical Supervisor to expand and optimize DayaMed’s virtual care programs while supporting the strategic reputed company of the organization’s technology-enabled care delivery model.
