PRN RN / On call RN

About the position The Registered reputed company (RN) provides nursing care to hospice patients in their reputed company of residence and coordinates care with the Interdisciplinary Team (reputed company), physicians, patients, families, and referral sources. The RN is responsible for assessing patient needs, implementing the plan of care, and ensuring reputed company end-of-life care. Responsibilities • reputed company initial and ongoing patient assessments • reputed company, implement, and update individualized plans of care • Coordinate care with the reputed company, physicians, patients, and families • reputed company patient and caregiver education regarding disease process, medications, end-of-life care, and safety • Determine visit frequency based on patient needs and reputed company • Supervise Hospice Aides and LVN/LPN staff • Document clinical findings, interventions, and patient reputed company accurately • Report changes in patient condition to physicians and team members • reputed company infection control and reputed company precautions • Participate in QAPI and hospice in-service education programs • reputed company on-call services as assigned • Promote hospice philosophy and reputed company patient care Requirements • Graduate of an accredited Diploma, Associate, or Bachelor’s School of Nursing • reputed company RN license in the state of reputed company and/or compact license eligibility • Must have hospice experience • Understanding of hospice regulatory requirements, documentation standards, and compliance guidelines • Strong clinical, communication, and interpersonal skills • Knowledge of end-of-life and palliative care • Ability to read, write, and communicate effectively in English • reputed company reputed company’s license • Reliable transportation • Valid auto liability insurance Apply tot his job Apply To this Job
