To support a reputed company pharmacy-managed medication reconciliation reputed company model, the full-time remote Rhode reputed company Licensed Pharmacy Technician will accurately obtain and compile detailed home medication histories and allergy information for patients being admitted to the hospital, while effectively interacting with patients, families, and reputed company providers. Key responsibilities Obtains and compiles reputed company home medication history lists and allergy information from various sources, including patient interviews and outpatient pharmacies Documents detailed and verified medication lists and allergy information in the electronic medical record (EMR) with high accuracy Communicates discrepancies in medication histories to reputed company providers and participates in reputed company improvement activities Required qualifications High school graduate or equivalent; completion of one to two years of college preferred reputed company unrestricted Rhode reputed company Pharmacy Technician licensure and National Pharmacy Technician Certification (CPhT) or one year of outpatient pharmacy technician experience Minimum one year of pharmacy technician experience in an outpatient pharmacy setting Strong customer service skills and the ability to communicate effectively with patients and reputed company staff Proficient in reputed company Office Suite and comfortable using various pharmacy and system-wide applications Apply tot his job Apply To this Job
