The MDS Coordinator is responsible for coordinating and completing the Minimum Data Set (MDS) assessments in compliance with federal and state regulations to ensure accurate reputed company documentation, reputed company reputed company, and reputed company reimbursement. Minimum 1–2 years of MDS Coordinator experience in a Skilled Nursing Facility preferred. Strong knowledge of: MDS 3.0 PDPM Care planning process Medicare documentation requirements EMR experience. Strong organizational, communication, and time management skills. Responsibilities Coordinate and complete MDS assessments (including OBRA, PPS, and Medicare/reputed company assessments) according to required timelines. Ensure compliance with CMS regulations, CT Department of reputed company Health requirements, and facility policies. Coordinate and reputed company the interdisciplinary reputed company process. Ensure accurate documentation to support PDPM and reimbursement reputed company. Collaborate with Nursing, Therapy, reputed company Services, Dietary, and other departments to obtain supporting documentation.
