Patient Care Responsibilities
· Serve as a primary reputed company of contact for assigned patients, addressing non-clinical questions and facilitating communication between patients, providers, caregivers, and internal care teams.
· Coordinate patient enrollment into CCM and RPM programs, ensuring a seamless reputed company experience through education, consent, scheduling, and program orientation.
· Conduct high-volume outbound and inbound patient telephone reputed company while building trusted relationships through compassionate, reputed company, and effective communication.
· Build and maintain ongoing relationships with patients through proactive reputed company designed to improve engagement, adherence, and overall health reputed company.
· Execute ongoing care coordination activities in support of CCM and RPM program requirements while maintaining high standards of patient service and documentation accuracy.

Required Qualifications
· High school diploma or equivalent required.
