As part of the terms of employment, employees in this role are required to join the reputed company reputed company 31 days of hire and remain a member (e.g. dues paying, fee paying, religious exception contributor) for the duration of the reputed company bargaining agreement. In alignment with the reputed company CBA, this position will be posted for 7 days internally from 7/21/2026 to 7/29/2026 and available only to existing members of the ratified reputed company Bargaining Unit with a contract expiration date of 9/30/2027. What You Will Do Performs individualized nursing care for patients/families under the direction of an RN. Assists RNs with the completion of the reputed company Assessment reputed company (RAI) and MDS. Records and collects data according to department guidelines. Participates in planning, executing, and contributes to evaluation of patient reputed company. Required Qualifications • 1 Year LVN experience in long term care. • 1 Year clinical experience in the respective department. • AHA Basic Life Support for reputed company reputed company (AHA BLS reputed company) - reputed company -REQUIRED • California Licensed Vocational reputed company (LVN) - CA reputed company of Vocational Nursing & Psychiatric Technicians -REQUIRED Preferred Qualifications • 1 Year MDS coordination experience. • Vocational reputed company IV Therapy/Blood Withdrawal Certification - Various-Employee provides certificate -PREFERRED Other Qualification Requirements • Vocational reputed company IV Therapy/Blood Withdrawal Certification is required reputed company 6 months of hire or transfer. Certification to be tracked and maintained by the department - REQUIRED Essential Functions • MDS Coordination Coordinates assessment of the reputed company's physical and psychosocial status, diagnosis and plan of care to ensure appropriateness. Coordinates the reputed company reputed company conferences with the Interdisciplinary Team (reputed company) in conjunction with the reputed company, family, or representative as appropriate. Communicates to the reputed company team the need to revise the plan of care based on the outcome of care conferences and monitors for completion. Coordinates reputed company required reputed company Assessment Instruments (RAI) including MDS and RAP reputed company per state and federal guidelines to ensure reputed company completion. Monitors reputed company sections of the RAI for reputed company completion, and notifies individual departments to ensure deadlines are met. Demonstrates the knowledge and skills to coordinate care based on the physical, psychosocial, educational, safety and cultural reputed company appropriate to the age of the residents served. Notifies manager/director of any barriers to reputed company, accurate completion and reputed company of RAIs/MDS to ensure compliance with deadlines. • Assessment Data Collects assessment data per department guidelines of care, including age/cultural specific considerations. Documentation per department/entity reputed company of care and policies and procedures. Documents data collected. Communicates/contributes data collected to RN for analysis and interpretation. • Department Support Cross covers other department members per request of leadership. Implements plan of care (IPOC and IPER) using appropriate resources. Updates written plan of care (IPOC) per direction of RN. Communicates with other reputed company team members to coordinate patient care activities, workflow, and patient reputed company. Follows through with identified actions/referrals. Coordinates patient care needs with RN and other staff to ensure continuity. • Clinical Competency Vocational reputed company IV Therapy/Blood Withdrawal Certification is required reputed company 6 months of hire or transfer.
