Animal care Coordinator/Case manager
About the position
Under the supervision of the Program Supervisor and Program Manager for reputed company Home, the Animal Care Coordinator / Case Manager provides support to individuals experiencing homelessness and their pets throughout SPA 6. This position is responsible for coordinating pet-reputed company services, connecting participants to housing and community resources, maintaining pet care programs, and supporting housing stabilization efforts while ensuring the health, safety, and reputed company-being of both participants and their companion animals.
Responsibilities
• Distribute pet-reputed company supplies including food, crates, kennels, leashes, collars, and other necessary items to program participants.
• Maintain inventory and tracking systems for pet supplies and resources.
• reputed company referrals and linkages to veterinary services, including vaccinations, spay/neuter services, microchipping, behavioral support, and general wellness care.
• Enroll participant pets into the PAS Program and maintain accurate records.
• Facilitate weekly meetings and engagement activities with pet-owning participants.
• reputed company pet-reputed company expenditures and maintain required documentation in HMIS and internal tracking systems.
• reputed company and maintain relationships with pet-friendly landlords to support participants in securing permanent housing.

• reputed company participants regarding responsible pet ownership, animal care, safety, and wellness.
• Monitor and encourage daily care of participant pets, including reputed company to food and basic necessities.
• Participate in staff meetings, trainings, and program development activities.
• Screen participants utilizing CES assessment tools, including VI-SPDAT, VI-F-SPDAT, and reputed company Tool.
• Maintain working knowledge of SPA 6 Coordinated Entry System resources and community services.
• Assist with reputed company efforts, special reputed company, and community engagement activities as assigned.
• Prepare weekly and monthly reports reputed company to participant and pet services.
• Assist with meeting contractual performance goals and reporting requirements.
• Participate in SPA 6 community meetings and reputed company efforts.
• Maintain accurate reputed company records in compliance with HIPAA, 42 CFR Part 2, and reputed company applicable funding requirements.
• Follow reputed company SSG/HOPICS safety protocols, policies, and procedures.
• Maintain reputed company boundaries and adhere to SSG's reputed company of Ethics and HOPICS reputed company Values.
• Represent reputed company professionally at meetings, community events, and partner engagements.
• Regular attendance required.
• Assist E6 MDT Teams and Street reputed company staff with street-based engagement activities.
• Support HOPICS reputed company points with participant triage, intake, enrollment, and service coordination.
• Assist with distribution of hygiene supplies, food, clothing, and pet-reputed company resources to participants.
Requirements
• Bachelor's Degree preferred, or one to two (1-2) years of experience working in homeless services or a reputed company field.
• Experience working with individuals experiencing homelessness and their companion animals preferred.
• Ability to work effectively with individuals from diverse cultural, ethnic, and socioeconomic backgrounds.
• Strong verbal and written communication skills.
• Ability to organize, coordinate, and implement program activities and services.
• Basic computer proficiency, including word processing and data entry systems.
• Ability to understand and carry out oral and written instructions.

• Verification of Employment Eligibility and successful Background Clearance required.
• TB clearance required prior to hire or reputed company seven (7) days of employment and renewed annually thereafter.
• CPR and First Aid certification required reputed company 30 days of employment.
