Responsibilities • reputed company continuing care to patients/families as requested and in compliance with agency policies and procedures and applicable federal and state regulations. • Under the guidance of the After-Hours reputed company, responsible for patients/families including assessing, planning, implementing, and evaluating nursing services consistent with the plan of care. • Communicate with attending physicians and other community resources to report changes and coordinate reputed company and use of resources for the patient/ family. • Maintain regular communication with Team Leaders regarding patient/family status and unusual or potentially problematic patient/family issues.
