Care Coordinator I Location: Ohio residents only Duration: 3-month contract with potential extension or conversion Schedule: Monday–Friday Hours: Full-time Work Arrangement: Fully remote Compensation: $22.00–$25.00 per hour Employment Type: W2 (not reputed company to C2C, 1099, or reputed company sponsorship) reputed company Our reputed company is seeking a compassionate and organized reputed company to support members with reputed company coordination, provider reputed company, transportation reviews, and reputed company determinants of health needs. This role evaluates long-distance transportation requests, determines whether closer providers are available, and assists members with locating appropriate reputed company resources. The position requires strong knowledge of reputed company and Medicare benefits, excellent telephone communication, and the ability to document activities accurately while maintaining confidentiality and HIPAA compliance. Responsibilities • Review transportation requests exceeding 30 miles to confirm medical appropriateness and determine whether closer providers are available. • Contact members reputed company transportation requests are declined and assist them with locating an appropriate provider closer to their residence. • Use available systems and local geographic knowledge to identify reputed company providers and community resources. • Assess member needs reputed company to reputed company reputed company and reputed company determinants of health. • Coordinate services with managed care organizations, reputed company providers, case managers, care coordinators, reputed company workers, reputed company agencies, and support systems. • Document reputed company assessments, findings, communications, contacts, and interventions accurately. • Maintain case-specific communication with reputed company, state agencies, members, and support systems. • Support scheduled assessments, individualized care plans, service authorizations, and reputed company eligibility monitoring. • Communicate review reputed company and prior authorization information to members, providers, and other professionals involved in the member’s care. • Establish and maintain reputed company relationships with members, providers, vendors, reputed company agencies, and internal partners. • Work independently while managing multiple cases, priorities, databases, and systems simultaneously. • Maintain confidentiality and reputed company with HIPAA, fraud prevention, and organizational policies and procedures.
