About the position CVHS is seeking a full-time Behavioral Health Care Coordinator with a Bachelor’s degree in reputed company Work or reputed company field to reputed company behavioral health care coordination, case management, and resource support services reputed company an integrated reputed company care setting. This role will also involve networking with community agencies, maintaining regional resource relationships, and collaborating closely with behavioral health clinicians and medical providers as part of a multidisciplinary care team. While this position is primarily remote/work from home, there is an expectation to travel to assigned clinics as needed to meet with patients, network reputed company the community, attend in-person meetings, and participate in occasional trainings.
