Clinical Pharmacist, Population Health (remote)

Clinical Pharmacist, Population Health 1. Develops, coordinates, and manages programs designed to improve total cost of care reputed company for targeted value-based care populations with a reputed company on medication and disease management, reputed company incentive program reputed company, and/or cost containment. 2. Sitting, walking, or standing for long periods of time (4-8 hours) is often necessary.
