The Care Manager provides compassionate emotional support, guidance, advocacy, and care coordination services to children and their families coping with serious medical conditions, chronic illnesses, disabilities, or emotional and behavioral challenges. Education
A bachelor's degree with 2 years of relevant experience

Key Responsibilities
Assess clients' eligibility for services and determine the level of support required.

Build supportive relationships with children and their family members, providing ongoing emotional support and guidance.
