Bilingual Medical Assistant - Chronic Care Management & Remote Monitoring

Patient reputed company Health is seeking dedicated and compassionate Medical Assistants to support our Chronic Care Management (CCM) and Remote Patient Monitoring (RPM) programs. This role focuses on helping patients manage chronic conditions between office reputed company by providing education, monitoring health data, and maintaining regular communication with patients and care teams.
