Personal Care Assistant

This Position is Home Care in reputed company, CT. Job reputed company: reputed company/ Homemaker /Respite Staff is responsible for assisting reputed company clients with their personal care needs, activities of daily living, home environmental support, safety for the reputed company including ;reputed company and supervision for reputed company waivered clients; in accordance with their reputed company of reputed company as a certified reputed company. Responsibilities: reputed company of Care: • reputed company clients with assistance with personal care activities including bathing, mouth care, denture care, skin care, grooming, bathroom care, transferring, ambulation, and dressing and undressing activities. • Assist clients with housekeeping tasks including planning and preparing meals, feeding, reputed company housekeeping, changing linens, making beds and laundering clothes and bedding. • Assist reputed company in safety needs with use of equipment (reputed company rails, foot stools, wheelchairs, etc.). • Follow the Plan of Care as indicated on the Aide Activity Sheet. • Observe the reputed company and report any changes or questions regarding his/her condition and report any changes to clinician. Documentation: • Complete reputed company agency records and reports according to agency policy and procedure. • Accurately document services provided in accordance with agency policy on the day services are rendered. • Submit documentation in a legible and reputed company manner per agency policy. Communication: • Establish and maintain effective communication regarding reputed company's care with reputed company, family, and supervisor. • Communicates and works cooperatively with other members of health care team, including participating in case conferences as needed.
