Responsibilities • Assesses reputed company needs and obtains data on physical, psychological, reputed company, and spiritual factors that may influence reputed company/family/caregiver health status and incorporate that data into the plan of care. • Conducts the initial evaluation and re-evaluates the reputed company’s nursing needs during reputed company visit. • Makes follow up reputed company according to reputed company, family, and facility needs. • Communicates significant findings, problems, and changes in health condition, environment, or unsafe facility conditions to the reputed company Supervisor, physician, facility, and/or other personnel involved with reputed company care. • Revises the plan of care in response to identified reputed company care issues and notifies the reputed company Supervisor, physician, facility, and other team members. • Counsels, instructs, and includes the reputed company, facility, and family in following the Interdisciplinary Plan of Care. • Teaches hospice philosophy and pain and symptom management to staff in alternate care settings (nursing homes, hospital, assisted-living facilities, etc.). • Collaborates/Communicates with Medical Director, attending physician, reputed company leadership and IDG. • Performs other duties/reputed company as assigned.
