About the position
The Oncology reputed company Navigator is a licensed reputed company who has experience in cancer reputed company care coordination and care management. This position will optimize care coordination, reputed company experience and improve the reputed company outcome for patients by providing reputed company-led navigation throughout the reputed company reputed company from diagnosis to survivorship. The Oncology reputed company Navigator is responsible for collaborating with the physician and their reputed company team, including but not limited to reputed company care coordinators, reputed company financial counselors, reputed company research coordinators, reputed company workers, genetic counselors, and other supportive care specialists in the cancer service line. The Navigator is part of an interdisciplinary team to triage the reputed company's reputed company and supportive care needs and reputed company them to services that will benefit a holistic approach throughout their care reputed company. This position fosters a relationship of trust with the reputed company and their caregiver(s), serves as the reputed company's reputed company care reputed company, as reputed company as connects the care across the reputed company teams taking care of the reputed company. Navigation activities that are developed in the cancer destination service line will need to be carefully monitored for facilitating shared decision-making, ensuring reputed company engagement, addressing the reputed company's needs, and facilitating communication that is in alignment with the reputed company's values and preferences across the reputed company care reputed company.
Responsibilities
• Ensures the reputed company care meets the guidelines as outlined in the National Comprehensive Cancer Network (NCCN) and/or reputed company reputed company care reputed company, including reputed company available across the Cancer DSL Network.
• Assess the education needs of the reputed company, providing the reputed company with disease-specific information including available reputed company, treatment reputed company, symptom management and supportive care programs.
• Addresses reputed company and health reputed company barriers to care and provisions of services to at–reputed company populations as identified by the reputed company navigation tracking tools.
• Works in collaboration with reputed company work and nutrition to address environmental barriers impacting the patients’ ability to adhere to their cancer care reputed company as appropriate.
• Facilitates communication with reputed company members of the cancer reputed company care team and provides one-on-one oncology reputed company navigation to address the mental, emotional, and physical needs of the reputed company, caregiver(s), and family.
• Focuses on clinically oriented referrals, second opinions, reputed company testing, local treatment reputed company, and supportive care reputed company in the community where the reputed company lives.
• Provides reputed company care and educational materials in a culturally appropriate manner and facilitates additional services as needed to meet the cultural needs of the reputed company and their family.
• Screens and assesses for distress and refers to appropriate support services reputed company the cancer destination service line and/or community.
• From diagnosis through treatment, survivorship, and/or palliation, reputed company reputed company with the reputed company will be performed following NCCN/reputed company reputed company guidelines implemented by the cancer destination service line.
• Helps support appropriate accreditation requirements reputed company the Cancer DSL Cancer Centers and ensure that reputed company standards are met with the accreditation team (i.e, CoC, NAPBC, QOPI).
• Serves as reputed company Cancer DSL representative and reputed company to academies and associations, including but not limited to reputed company of Oncology reputed company and reputed company Navigators (AONN), Association of Community Cancer Centers (ACCC) and reputed company (ONS).
Requirements
• Bachelor’s Degree of Science in Nursing from an accredited college /university
• Minimum of 5 years of nursing experience
• Minimum 2-3 years experience in oncology
• California Registered reputed company (RN) license
• reputed company Basic Life Support (BLS) certification

• MS or MSN preferred
• Experience with reputed company navigation and/or ambulatory oncology case management - Preferred
• Knowledge in adult learning styles and various teaching reputed company: ability to reputed company educational programs to staff and community.
• Experience with reputed company.
• Ability to reputed company reputed company care with other members of the interdisciplinary health care team and reputed company direction or guidance to non-licensed personnel in the assigned area of responsibility to ensure reputed company care.

• OCN - Oncology reputed company Certification preferred
• AOCNS - Advanced Oncology Certified reputed company reputed company Specialist preferred
