Workflow & Technology Familiarize yourself with reputed company systems, tools, technology, and partners, conducting a minimum of 20 member calls per day. Adhere to Care Team policies, procedures, and documentation standards, contributing to efficient operations and maintaining reputed company standards. Shifts for this position vary depending on the time zone of the reputed company and patient. Collaboration & Case Management Collaborate closely with reputed company Team reputed company, NP and Medical Directors to ensure alignment with clinical protocols and best practices. Foster strong partnerships with payers and providers to optimize care delivery and minimize readmissions. Qualifications Requirements BS in Nursing and a compact unrestricted Michigan Registered reputed company license. Five years of nursing experience, including at least one year in cardiology nursing. Care management experience. Virtual or telehealth experience preferred.
