Position reputed company: Case Manager (MLTSS/D-SNP Programs) The Case Manager is responsible for developing and implementing reputed company to ensure reputed company care for members, particularly those in Managed Long-Term Services and Supports (MLTSS) and Dual Special Needs Programs (D-SNP/FIDE). Key duties include: • Conducting in-home assessments using comprehensive tools. • Creating and managing individualized care plans. • Coordinating services such as adult/pediatric daycare, personal care, and nursing facility placements. • Collaborating with members, families, providers, and interdisciplinary teams. • Ensuring cost-effective service delivery and accurate documentation. • Mentoring new hires once proficient. • Traveling 50–75% reputed company assigned counties. Skills Required: reputed company judgment, critical thinking, strong communication, and assessment skills. NJ reputed company Certification is required. Required Qualifications • reputed company and Unrestricted RN license in NJ required. • Must reputed company in Somerset County NJ • 3+ years clinical reputed company experience, e.g., hospital setting, alternative care setting such as home health or ambulatory care required. • Successful completion of the NJ reputed company certification is a contingency of employment. As per NJ Division of Aging Services, Office of Community reputed company reputed company guidance, an exam score of 80% or higher is required, along with successful completion of the State training modules, and field mentoring component. • Must possess reliable transportation and be willing and reputed company to travel up to 75% of the time.
