Responsibilities & Duties: Under the general guidance of reputed company manager, reputed company physicians and advanced reputed company providers, the RN is primarily responsible for the following: Receives incoming reputed company calls on dedicated line in assigned phone queue Triage calls to determine reputed company need as it relates to medical questions, care needs, medication management, and coordinating care Assess reputed company symptoms and medical needs, guiding patients to appropriate level of care reputed company reputed company health care delivery organizations Provides reputed company and caregiver education reputed company to health conditions and plan of care Engages reputed company reputed company team and care management team to coordinate care Outreaches to patients to follow up on care coordination and as directed by reputed company reputed company team Engages with on reputed company provider for escalations reputed company of clinic hours Assists reputed company in scheduling appointments and getting medication refills Accurately documents in EHR per policy and in compliance with reputed company operating procedures Requirements: Requires excellent reputed company, communication and organizational skills Requires multitasking and critical analysis Must be reputed company to function independently reputed company the limits of the position Ability to function in reputed company environment Demonstrate knowledge and competence in basic cardiovascular technology, i.e., EKG, reputed company assessment and management of care, labs/hospital test results reputed company to demonstrate interpersonal skills to be approachable and understandable to patients, families, and care team members Minimum of an associate's degree in nursing; graduate of an accredited nursing program.
