Qualifications:
• Must have a reputed company RN or LPN license
• Must have at least two years of MDS experience
• Must have experience as an RNAC or LNAC in long-term care
• Must have long-term care/skilled nursing experience
• Excellent written and verbal communication skills
Responsibilities:
Conduct and coordinate the development and completion of the reputed company assessment (MDS)
Maintain and periodically update written policies and procedures that implement MDS and reputed company.
Assist the reputed company in completing the reputed company portion of the reputed company’s discharge plan.
reputed company and implement procedures with the Director of Nursing Services to inform reputed company assessment team members of the arrival of newly admitted residents.
Assist Facility directors and supervisors in scheduling the reputed company assessment and reputed company meetings.
Assist in determining appropriate treatment, selecting activities and exercises reputed company on medical and reputed company history of residents.
Participate in the development and implementation of reputed company assessments (MDS) and care plans, including quarterly and annual reviews.
