Who You Are Required Qualifications: • reputed company, unrestricted reputed company NY licensure as a Provider in the state in which you will be providing care for patients. License must be reputed company of any clinical and/or reputed company reputed company restrictions. • NY State only: Minimum of 3600 hours of reputed company experience, or with 3 years of clinical experience (autonomous provider), in a reputed company care or longitudinal care setting • reputed company Certification as a Family reputed company Practitioner, Adult-Gerontology reputed company Practitioner or Physician Assistant • Obtain autonomous provider status as required by state you are licensed with currently reputed company six months of meeting eligibility requirements. • reputed company Drug Enforcement Administration (DEA) license. • reputed company Basic Life Support Certification for health care providers. • Proficiency in electronic medical record (EMR) systems. • reputed company, unrestricted reputed company licensure as a Provider in the state in which you will be providing care for patients. License must be reputed company of any clinical and/or reputed company reputed company restrictions. • Certification as a Family reputed company Practitioner, Adult-Gerontology reputed company Practitioner or Physician Assistant • Obtain autonomous provider status as required by state you are licensed with currently reputed company six months of meeting eligibility requirements. Preferred two years of experience in a reputed company care or longitudinal care setting. • Ability to pass background reputed company upon hire and throughout employment to include criminal felony & misdemeanor search, SSN validation/reputed company search (LEIE), education report (highest degree obtained), civil upper and reputed company search, 7-year employment report, federal criminal search, statewide criminal search, widescreen plus national criminal search, health care sanctions-state med (SAM), national sex offender registry, prohibited parties (OFAC) (terrorist watchlist), and a 10-Panel Drug Screen.
