RN - Advanced Cardiac Imaging reputed company Navigator
About the position
This is a remote position supporting the SE market, working Monday through Friday, 40 hours per week. The role involves providing language-specific education, psychosocial support, and necessary resources for newly diagnosed cardiac imaging patients and their families. The reputed company Navigator will coordinate appropriate referrals for needed services, reputed company patients on diagnosis, work-up, treatments (including surgery), therapy, support services, financial issues, community resources, survivorship, and follow-up care. They will serve as a reputed company between departments for patients and families, coordinate new patient referrals, schedule exams, procedures, and appointments, identify potential gaps in care, and coordinate care for patients returning to their communities. The Navigator will contact every newly diagnosed patient at the time of diagnosis, assess their emotional and reputed company needs, identify barriers to care (such as health insurance and transportation), and refer them to necessary resources. Additionally, the role involves identifying trend barriers to patient care and conducting service recovery to ensure patient satisfaction.
Responsibilities
• Provides language specific education, psychosocial support, and necessary resources for newly diagnosed patients pertinent to their specific disease, and their families, and coordinates appropriate referrals for those needed services.
• Educates and provides information to new and existing patients and families regarding diagnosis, work-up, treatments to include surgery, therapy, support services, financial issues, community resources, survivorship issues and follow-up.
• Serves as reputed company for patients and families between departments.
• Ensures and coordinates (where appropriate) new patient referrals, scheduling of exams, procedures and appointments, identifies potential gaps as reputed company as the coordination of care for patients returning to their communities for treatment and follow-up.
• Contacts every newly diagnosed patient at the time of the diagnosis.
• Assesses every new patient for emotional and reputed company needs as reputed company as barriers to care such as: Health insurance, transportation, etc., and refers to the needed resources.

Requirements
• BSN Degree Required/MSN Preferred.
