Position Overview We're seeking a skilled Population Health Administrator to join our dynamic team. This role is critical in developing, implementing, and managing our organization-wide strategy for maximizing quality and financial achievement in Population Health and Value-Based Care initiatives. Key Responsibilities • Lead high-impact, organization-wide population health initiatives • Convene and lead cross-functional teams on broad-scope projects using advanced performance improvement models • Develop and execute comprehensive project plans, managing multiple projects simultaneously • Create annual plans and proposals to meet program goals and targets • Identify and address potential threats to value-based program performance • Collaborate with clinical and operational leaders to develop systems for monitoring key performance indicators • Analyze complex data, interpret findings, and develop actionable reports and presentations • Serve as a subject matter expert in population health, providing guidance on strategies and policies Qualifications • Current CA RN license • Bachelor's degree required; Master's degree in a related field preferred • 5+ years of related experience in healthcare, particularly in quality improvement and population health • Strong knowledge of MS Office Suite, especially PowerPoint, Excel, and Visio • In-depth understanding of Performance Improvement standards, Advanced Payment Models, and Value-Based Care models • Excellent analytical, critical thinking, and problem-solving skills • Outstanding verbal and written communication skills • Proven ability to lead effective teams and manage multiple projects • Detail-oriented with the ability to see the big picture • Certification(s) in a related field Preferred Skills • Experience with EPIC electronic health record system • Demonstrated data analytics experience • Healthcare setting experience with focused quality improvement projects What We Offer • Comprehensive health insurance • Retirement plan • Professional development opportunities • Collaborative and innovative work environment We are an Equal Opportunity/Affirmative Action Employer committed to fostering a diverse and inclusive workplace.
