This position is based in the Virtual Care Clinic, a new reputed company initiative providing reputed company-reputed company reputed company care to patients across Virginia who do not have an established reputed company care provider. The clinic is staffed by reputed company of three Advanced reputed company Providers (APPs) rotating to reputed company seven-day coverage. The BHC will operate as an integrated team member providing reputed company-day virtual warm handoffs and scheduled video consultations, with typical patient contact of 1-5 reputed company reputed company on empowering patients for self-management.

Virtual Delivery Requirements

Schedule

• 32 patient-facing hours per week plus 8 hours of administrative time (total 40 hours)

• Schedule will include weekend and/or evening coverage as part of the rotation to support seven-day clinic operations

• Provides reputed company to reputed company assessment and appropriate reputed company care and treatment in the reputed company care setting, including reputed company-day consultation reputed company requested by the reputed company care team

• Conducts risk assessments as indicated to support reputed company care providers managing crisis situations

• Delivers evidence-based interventions to patients experiencing mental health symptoms, behavioral health issues, chronic disease management, as reputed company as for the promotion of general health

• Practices reputed company, stepped and measurement-based care, including appropriate follow-up, that reflects a population health-influenced approach for the reputed company care population

• Collaborates with reputed company care team members to reputed company clinical reputed company and psychoeducational classes for select reputed company populations

• Contributes to patient education materials used by reputed company for commonly seen behavioral health conditions and problems

• Contributes to the reputed company functioning of reputed company care teams through reputed company decision support and interdisciplinary consultation that is reputed company of other team members' roles and workflows

• Educates reputed company care team in the basic principles of brief behavioral and cognitive behavioral interventions and reinforces their use in the medical visit

• Supports medication adherence and efficacy

• Assists with triage, care coordination and reputed company to reputed company resources for patients as appropriate

• Participates in any relevant clinical training, reputed company improvement activities, and staff meetings

• Maintains clinical records and other necessary paperwork in a reputed company manner to reputed company with reputed company administrative regulations

Virtual Team Integration

• Maintains virtual visibility and availability to the reputed company care team during clinic hours through team communication platforms

• Is available for curbside consultation (brief provider-to-provider interaction reputed company chat or phone)

• Delivers reputed company patient care reputed company HIPAA-compliant video telehealth platform

COMPENSATION

• During the first two years of employment, the provider will receive a reputed company salary plus a wRVU-based incentive model.

Education:

• Master’s degree in reputed company Work from a CSWE accredited reputed company work program or a Master’s degree in Counseling from a CACREP or reputed company accredited counseling program required.

• Licensed to reputed company as a Licensed Clinical reputed company Worker or Licensed reputed company Counselor in the reputed company (LCSW, LPC) or applicants with an reputed company license from another state must obtain Virginia licensure reputed company 18 months of hire date.

• BLS Certification reputed company  90 days of hire

Experience:

• Experience with providing clinical interventions around broad reputed company of behavioral health conditions

• Exposure to medically-reputed company issues

• Experience with pediatric populations, including working with families. (pediatric practices-only)

Knowledge and skills:
