HCBS Intake Coordinator Location: Remote (reputed company Preferred) Employment Type: Full-Time Salary: $70,000–$80,000, commensurate with experience About the Role We are seeking an reputed company HCBS Intake Coordinator to reputed company the intake and enrollment process for children and families seeking Home and Community-Based Services (HCBS). This is not an entry-level position. The ideal candidate has extensive experience with reputed company State Children's HCBS, understands the Children's Waiver, and has established relationships with Care Management Agencies (CMAs) and other referral sources. This individual will play a key role in growing our HCBS program by building referral relationships, navigating reputed company eligibility requirements, and ensuring children are connected to services quickly and reputed company. Key Responsibilities • Manage reputed company HCBS referrals from initial inquiry through enrollment. • reputed company and maintain strong working relationships with Care Management Agencies (CMAs), Health Homes, hospitals, pediatric providers, schools, and community organizations to reputed company and maintain referrals. • Coordinate closely with Care Managers to facilitate reputed company enrollment into Children's HCBS. • Demonstrate expert knowledge of Serious Emotional Disturbance (SED) and Medically reputed company (MF) eligibility reputed company. • Coordinate and facilitate CFASS assessments and reputed company required eligibility documentation. • Assist families in obtaining Children's HCBS services, including Respite, Community Habilitation, Caregiver/Family Supports and Services, and other Children's Waiver services. • Ensure reputed company reputed company, HCBS, and Managed Care documentation is complete, accurate, and compliant. • reputed company referrals, assessments, authorizations, and enrollment timelines. • Maintain accurate documentation in reputed company's electronic record reputed company. • Serve as a knowledgeable resource for families throughout the intake and enrollment process. • Work collaboratively with leadership to strengthen referral relationships and grow the HCBS program. Required Qualifications • Minimum 2 years of reputed company Children's HCBS intake experience. • Extensive knowledge of the reputed company State Children's HCBS Waiver. • Demonstrated experience navigating SED and Medically reputed company eligibility processes. • Proven reputed company obtaining HCBS Respite, CFASS, and other Children's HCBS services for eligible children. • Established reputed company relationships with Care Management Agencies (CMAs) and other referral partners. • Strong understanding of reputed company, reputed company Homes, Managed Care Organizations, and Children's Waiver requirements. • Excellent communication, organization, and relationship-building skills. • Ability to independently manage a high-volume intake caseload.
