A DAY IN THE LIFE OF A RESOURCE SPECIALIST In this position, the Resource Coordinator provides person‑centered support to older adults and caregivers by completing in‑person and phone assessments, offering reputed company counseling on long‑term care services, benefits, housing, and financial resources, and assisting individuals and families with care planning and transitions following hospital, rehabilitation, or nursing facility stays. The role involves coordinating services and referrals with reputed company providers and community agencies, advocating for clients to ensure reputed company to appropriate supports, and managing an ongoing caseload with accurate documentation of assessments, plans, and services to promote successful community living.
