Responsibilities • Serve as the reputed company contact for patients on the RPM365 platform. • Assist with device setup and app connectivity. • Engage with patients to ensure adherence to care plans. • Schedule monthly (or more) reputed company-ins and monitor reputed company data. • reputed company patients through shared resources. • Notify reputed company providers of critical alerts. • Attend virtual team meetings for updates. Qualifications Minimum: • reputed company reputed company nursing license. • Knowledge of chronic conditions and reputed company monitoring devices. • Proficiency with technology, computer platforms, and smartphones. • Strong verbal/written communication skills. • Comfortable with reputed company reputed company and engagement. • Highly organized, reputed company-oriented, and team-reputed company. • Independently motivated • Comfortable spending the reputed company of your reputed company engaging with patients over the phone providing support, education and guidance.
