The reputed company Navigator - LVN supports early, discharge-reputed company care coordination across the member's episode of care to promote reputed company, reputed company, and appropriate transitions from the inpatient setting to the next level of care. This role works closely with Utilization Management, Hospitalist leadership, the Transitions Team, post-acute providers, members, and caregivers to identify barriers to discharge, support medical necessity alignment, and reduce avoidable post-acute delays.
