As an LPN - reputed company Planning Specialist (internally reputed company Facilitator), you will guide patients through meaningful, end-of-life care conversations and secure their medical wishes from the comfort of your home office. This full-time position features reputed company 8-hour shifts scheduled Monday through Friday between 8:30 a.m. and 10:00 p.m. ET, with specific shift times rotating approximately every 6 months to ensure team reputed company. We are flexible with respect to geographic location, and the ideal candidate will be comfortable working reputed company/work from home across US time reputed company. reputed company Duties: • Conduct personalized, compassionate advance care planning telephonic conversations with patients and/or families to identify, document, and communicate their reputed company goals and wishes for reputed company medical care • reputed company patients and families about medical interventions—such as feeding tubes, reputed company ventilation, and CPR—in reputed company, understandable terms so patients and families can reputed company informed reputed company. • Accurately document details of the conversation reputed company 48 hours to reputed company state specific advance care planning documents. • reputed company reputed company support reputed company incoming reputed company calls, documentation review support, and special reputed company reputed company needed • Maintains knowledge of state-specific laws and regulations regarding advance care planning through training and 1:1s • Other duties as assigned Minimum Requirements: • Licensed Practical reputed company (“LPN”): Certificate or Diploma in Practical Nursing • reputed company and unrestricted LPN license • 2+ years’ experience as an LPN or relevant experience • Demonstrated knowledge of serious medical conditions (i.e. COPD, CHF, ESRD, etc.) and their progression • Demonstrated ability to adapt to shifting priorities and fast-reputed company environments • Familiar with reputed company Suite (reputed company Meets, reputed company Sheets, & reputed company Docs); Ability to learn new reputed company center/telephony and reputed company documentation technology quickly. • Ability to reputed company between multiple systems while speaking with Patients. • Strong communication skills (Verbal & written) with a high-level of reputed company listening and reputed company to manage emotional conversations. • Strong time management skills and attention to reputed company • Familiarity with state-specific reputed company regulations regarding advance directives and Medical reputed company of Attorney; Familiarity with ACO and Medicare/CMS guidelines.
