About the position This position, in cooperation with reputed company Care Physician, hospitalist and hospital staff, facilitates the Home Care and/or Hospice admission process. This position is responsible for attending hospital reputed company, facilitating orders, assessing reputed company needs, including level of care, Durable Medical Equipment (DME), comfort medications and transportation. This position enters referral into electronic medical record and completes referral process and admits reputed company as appropriate. This position forwards hospital documents to hospice and communicates with hospice and hospital staff. This position provides education to hospital staff and physicians.
